Look Up Look Out

Report an incident or near miss

There are far more close calls than people realise — too often they go unreported through embarrassment or being brushed off. There is no shame in it. Sharing what happened helps us learn, spot patterns, and protect each other.

Photos that help

Useful photos include damaged poles, how cables sit relative to the building, or the working position — real-world context that words alone can't replace. You can add up to five images (JPG, PNG, WEBP, or HEIC, up to 10MB each).

Campaign reporting only

This form is for the campaign — awareness, learning, and showing the scale of the problem. It is not about selling anything or telling you how to do your job. It does not replace official reporting to your employer, a network operator, or regulators when the law or your job requires it. For immediate danger, follow emergency guidance on our safety page — including calling 105 when a job feels unsafe.

Location (optional)

Optional. If you add a postcode or pin and opt in below, we show an approximate point on the public campaign map — not an exact address. We do not use this for shrouding requests (that flow has its own form).
